Reforming a province

Apologies if this has been asked before. If you form a province, then lose the minimum region requirements for the province, is there still a way to reform it?

You have to retake the minimum region requirement.

Just to elaborate a bit more, if the province has even number of regions, you need to take half of them + 1. If the province has an odd number of regions, you need to take half of them rounded up (3 if 5, 4 if 7, 5 if 9, etc.).

Lastly, if the province has an even number of regions and you have already formed the province, you can afford to own only half of them (2 out of 4, 3 out of 6, etc.) and still maintain the province. However, once you lose another region in the province and need to reform the province again, you will again need half + 1.
